RFANS COVID-19 Survey 

RFANS is looking to gather information on the financial impact COVID-19 is having and will have on recreation facilities. We want to ensure government understands the effect closures and the impact revenue loss will have on reopening plans and operations. The questions will include the need for some financial data. Only one representative from each facility should complete this survey, but feel free to consult with others on the answers to questions, as needed.  Your responses will be kept strictly confidential; not linked to your identity nor facility.
